HNHB LHIN provides home care services to over 82,00 patients in this Local Health Integration Network (LHIN). In the past year, 1 in 17 people in the region required care. HNHB LHIN provides a variety of services, from offering support to school-age children to ensuring palliative care for the people in HNHB LHIN. The organization also facilitates placement to long-term care homes for those people who cannot live independently any more and whose care needs cannot be met in the community alone.
Since HNHB HNHB strives to provide excellent care for its patients, collaboration with partners is crucial. HNHB LHIN works collaboratively with external partners like 9 hospital corporations (22 sites – 13 Emergency Departments – 5 Urgent Care Centres), 1 Aboriginal Centre – 7 Community Health Centres, in 19 Primary Care Groups/Family Health Teams, 85 Long-Term Care Homes, 87 Community Support Services and 5 Residential Hospices.
